Maintaining adequate perfusion pressure is the goal when titrating vasopressors. The mean arterial pressure reflects the average pressure driving blood through the organs and is influenced by both blood pressure and cardiac output. In most critically ill patients, the target MAP is about 65 mmHg because this level generally supports adequate cerebral and renal perfusion without causing unnecessary excessive vasoconstriction. Pushing the target much higher, like 75 or 90 mmHg, can increase vasoconstriction and cardiac workload without proven extra benefit and may even reduce organ perfusion in some cases. A MAP around 50 mmHg is usually not enough to guarantee perfusion of vital organs, especially in patients with chronic hypertension.
